The humanitarian program is the means by which physicians, treating relatively small numbers of individuals diagnosed with onchocerciasis in hospitals and dispensaries, can obtain Mectizan by special request.* This mechanism was established to provide Mectizan to people in areas where:
a) onchocerciasis is relatively rare and less severe and thus mass treatment is not indicated, and
b) for various reasons mass treatment programs cannot reach all affected populations.

Instructions for downloading the Special Request form in MS Word:**
1. In Internet Explorer right click on the Special Request form or the Demandes Spéciales de Mectizan® and "save target as" on your computer.
2. Open in MS Word to complete the form.
3. Save the completed form and send it via email, mail, or fax to the Mectizan Donation Program using the contact information above.
*Mectizan® is not approved for use in the US, and as a consequence, we are unable to fulfill donation requests to treat onchocerciasis patients residing in the US. However, Stromectol®, Merck’s commercial version of Mectizan®, is approved for use in the US and is available commercially. Stromectol® is registered in the US for the treatment of intestinal Strongyloides stercoralis and onchocerciasis and can be obtained from a pharmacy with a prescription.
